How do I complete a survey or outcome measure?

Physitrack allows your healthcare practitioner to add a questionnaire or outcome measure to the exercise programme they assign to you. A questionnaire can appear at different times, even if you have already filled it in previously. This way, your practitioner can better understand your progress.

If your practitioner has prepared a questionnaire for you, it will automatically appear in PhysiApp. For the results to be as true as possible, it is important to answer the questions at that time.

Before answering the questions, read the instructions carefully. To answer questions, simply click or tap the correct answer followed by Next.

The answers you provide will be automatically reported back to your practitioner upon completion of the questionnaire. They will see them in their account, along with details of your adherence (if applicable).
